    So, My professor for fundamentals said that charting by exception means recording or documenting unusual circumstance or finding(s) per patient. I want to understand why this is a charting procedure? what's the importance of it? And based on other charting methods such as focus or soap, the pt's impairment is recorded which will also include an unusual co morbid factor or problem. So please, someone explain to me why this charting is necessary? and if it is necessary is what exact condition is it used please. Thanks in advance

  4. Hello, I will be starting Long Island University nursing program in the fall of 2014. I am so frightened due to the testimonials I've heard and read. I'm also frightened because I've been told more than twice how abstract i think. Please does anyone have an idea or a basis to how to start thinking critically especially for the purpose of nursing because i do not want a C+ at all and i do not want to be dismissed from the program. Thanks
