Ackley is the nursing diagnosis book that was required for my program. I was a life saver!!! Like everything she said above, a nursing diagnosis and interventions are something you as a nurse can do something about without needing an order.
We were always to taught that a nursing diagnosis had to be written as follows: Fluid volume deficit related to osmotic diuresis as evidenced by tachycardia, increased respirations....you get the picture. we just alway had to have nursing diagnosis r/t aeb
you need to know if there is a DNR and a POA for the son. Other than that just make the patient care plan as if you are helping him because until a professional interpreter arrives that is what you will have to do.
