Magnesium sulfate
Following up on my own question: How do you calculate the following (what is the formula) ? Medication is: 20 grams Magnesium Sulfate in 500 ml of D/W bag. Order is: Give 6grams (150 mls) loading dose via IV pump over 20-30min, then give 2gr/hr maintenance dose.

Yes, I believe that is exactly my problem (the period of time is not specified) The orders do not say over what period of time the bolus should be administered. We usually give the bolus over 20 or 30 min. My point is, we DO that, but the orders don't clarify that. So, I presume we need to ask the docs to be more specific in their orders. The other thing was, I was looking for a formula. But without the time period the formula cannot be completed, correct ? The order usually just states "give Magnesium Loading dose 6grams (or 4 grams) and Maintenance dose 2gr/hr". We have pre-set pumps to calculate the administration with "over-ride" tabs which are locked for the 6gram dose. So I was wanting a formula for my own safety. I have forgotten the formula :)

Medication is supplied: 20 grams Magnesium Sulfate in 500 ml of D/W bag. Order is 6grams (150 mls) loading dose via IV pump then 2gr/hr maintenance dose. Pt diagnosis is severe pre-eclampsia. Management: 1st hour q 15min BP's, Resp, continuous Pusle Ox. then q 1 hr BP q 1 hr DTR's q 1 hr respirations q 1hr urine output q 1 hr iv input q 2hr PO q 4 hr auscultations of breath sounds q 4 hr Temp. Mag levels as per MD orders or if resp below 12/min and /or dtr's absent. My question is at what rate does the Bolus dose of 6 grams go in ? and over what time period ?

Congenital Pneumonia
I have a new grandbaby 24 hrs old with congenital pneumonia. I am in the states, he is in the UK. Getting messages from my son which are very concerning to me. Last message:- "platelets gone down. Inflammatory levels gone up. New labs just drawn to see if it's worsening. Also want to do a lumber puncture. Might happen later tonight depending on staff...if specialist is available " Your impressions please ! Baby started grunting soon after birth. Is not on IV fluids. Has IV access in arm. Is on antibiotics. ( don't know what abx) Trying to breastfeed. Not latching well.....appears lethargic ( son describes him as gelled and quiet ) baby is with mom in the room. Mom is pumping colostrum. Baby goes to neonatal unit prn for labs. i am a L&D Nurse. Don't know too much about NICU. Please comment.

Plastic Surgery
WOW !!! Enlightening. I appreciate the diverse thoughts, attitudes and ideas. I wonder what the cost is? I know it varies greatly from provider to provider and the different options available. I am under the impression that it is +- $500.00 a syringe for the light fillers and one (usually) needs more than one syringe (obviously depending on what one is having done). And then as mentioned, it would need to be reapeated 2-3 times per year. So maybe $2000.00 to $3000.00 per year. ($160.00 to $250.00) per month ? I would call that relatively expensive as an ongoing expense. I believe everyone's priorities are different and everyone has an opinion which I respect. If my son/daughter or someone I love asks me for my opinion I would be reluctant to encourage it. I do believe if the person who wants it, believes it gives them more self-confidence - there may other solutions to that. If there really is a problem with disfiguirement or one feels one needs a breast augmentation or reduction or a nose job, I deem that different to the sense that the effects of natural aging is ugly. As was mentioned above by NREMT - P/RN ..... inner beauty shine through and has a MORE BEAUTIFUL effect than any fillers/botox/or spray tan can achieve. But to each his own. Still....last word.....Smile, be conent, be kind, love a lot, have peace in your heart, be patient and KNOW THE TRUTH. Beauty will shine through you and you WILL be beautiful. How sad to be consumed with not wanting to grow old and not wanting to look old. It is inevitable. Media is so powerful it is frightening !

New grad RN job with no connections?
I'm so sorry to hear this. A friend's daughter is in the same boat. A bit discouraging but keep going....as someone mentioned above, get your foot in the door "somewhere". take a job in any area jsut as a start. I also believe it is a good idea to go the HR depts in the hospitals and try to meet with the recruiters there. Find other nurses who are working and ask them to introduce you to the HR dept where they are working.

Thank you for your reply. I will suggest that. Maybe she will need that little seed planted. She is young and living with her parent at the moment. Not sure how open she is to moving. But you're right, that may be her only option. Her father is pushing her to take"any" job eg. working in a nursing home just to get started, and in his opinion, "have something to offer" as experience and earn at least something instead of sitting at home becoming frustrated. She is reluctant to do this. I have no idea what the solution is. They are asking me if I have any "influence" where I work. I do not. Except maybe speaking to HR and asking some questions.

Plastic Surgery
It seems like Plastic Surgery or procedures like Botox and "Fillers" are becoming quite common-place. Who has an opinion on this? I have a few issues with this and questions. 1) Can it become addictive ? The Botox and "Fillers" appear to be so. i.e. (addictive) in the sense that it is not permanent and as the effects fade the person wants/needs/ to have the procedures re-done and it seems like a never-ending cycle. 2) I see more and more younger women having these procedures. What image is being portrayed to their children? Young girls...teens and pre-teens are exposed to these procedures and I have heard a pre-teen talking about her mother's Botox procedure as casually as if she is discussing the fact that her mother is grocery shopping. Any comments?

What are the practices out there? It is sometimes impossible to monitor the Fhr during the placement of an epidural, especially with the patient sitting up, and leaning over, as is a very common positon for the pt. to assume for the placement. There are differing opinions amongst our staff. No-one seems to be quite sure what the "correct thing" is to do? Is there "a wrong or a right way" regarding monitoring or not monitoring during this time ?
