In my program we have just begun doing care plans and it's quite challenging for me. I've done a lot of reading and listening and think I'm getting better at understanding what I need to choose for a nursing diagnosis...my main issue is knowing what to put for the interventions. I feel like my knowledge isn't up to par and it's discouraging me. The pt I'm currently assigned to has acute bowel obstruction, so I decided to choose, as my diagnosis, Abdominal Pain, then realized that's not a NANDA, so I've thought of changing it to acute pain. Could I put r/t bowel obstruction aeb verbalization and grimacing?? Or am I not to include the medical diagnosis anywhere in my care plan? My biggest issue is the nursing intervention....I don't know what to do. I feel like I don't know how to ease the pain, so I have no idea what to put. Is my lack of knowledge the issue here?? Please provide some insight. Much appreciated.
