  1. Hello, I am currently a home care nurse. At how many weeks did you tell your employer you were pregnant? How did you communicate that message (email, phone, directly in person...)? My director lives out of state so I hardly see her (maybe 3 times a year). We usually communicate via email. I do have a supervisor in the office which I see once a week (I go to the office to hand in paperwork once a week). But again, my supervisor and I usually communicate via email. I am wondering if it would be appropriate to tell my news through email.

  4. hello all! i'm studying for my nclex and i am so confused on which meds to give to a patient with pancreatitis. One book says no morphine because of spasms and to give demerol. Another book says that these days morphine is actually okay to give...it also says dilaudid is okay to give. I was always taught no morphine, and to give demerol. Which one is it? I am so confused after reading several books/questions! Please help! Thank you for responding!
  5. this is my case study. please help this with nursing diagnosis. ive been stuck for 2 hours! Neuroleptic Malignant Syndrome: a 40 year-old male with a h/o schizophrenia is brought to the ER by the police after finding him running around in a field and wearing very little clothing. he physically resisted getting into the police care before being brought to the ER. he has not been taking his medication. he tells the triage nurse he is from another planet and there is a force field around him. as the CNA tries to put him in a hospital gown, he pushes her to the floor and strikes her in the face. the patient is ordered and given an IM medication cocktail of Haldol 5mg. atvan 2 mg. and benadryl 50mg. before being admitted to the psychiatric unit as an involuntary admission, he is given an oral dose of haldol 5mg. on the third day of admission the patient complains of muscle spams, difficulty breathing, and difficulty remembering where his room is. the nurse assigned to the patient obtains his vital signs: oral temp=105 F, pulse= 100, respirations=24,pulse ox=89% and BP=160/96. the patient has muscle rigidity, +3 deep tendon reflexes, and diaphoresis. the nurse notifies the psychiatrist and receives orders for stat lab test. CPK=1000, ALT/SGPT=100, AST/SGOT=80,=120, creatinine=0.8, BUN=20, WBCs=15. what do you think of this nursing diagnosis? non compliance related to increasing amt. of disease-related symptoms despite adherence to advise regimen as evidenced by muscle spams. non compliance related to barriers to comprehension secondary to schizophrenia as evidenced by poor memory. what are the subjective and objective assessments of this patient? thank you.
