  1. I use a clip board or a folder. If they're on contacts keep the board upside down outside of the room, like up at the nurses station. The folder is nice cause you don't have to worry about forgetting to turning it upside down to keep pts info safe. I know other students like to use a three ring binder. Also if you get a clip board that has an internal area for papers to stay organized is good too.
  2. I also agree with CT Pixie. Make your priority diagnosis based on your ABCs. When it comes to pt safety physiological interventions are considered priority over psychosocial.
  3. That would be a good psychosocial diagnosis. You could also use Hopelessness. Death Anxiety, Impaired Gas Exchange d/t chemotherapy drugs, Nausea, Imbalanced Nutrition: Less than body req., d/t depression and chemo therapy... There's a lot I could write down lol. If you're focusing on a specific area such as the depression, cancer or VRE let me know and I try to write back and be more specific :) Hope this helps!
