  1. I'm on a PCU floor and my clinical instructor wants us to have our binders so organized, and have all cardiac meds on a chart with nursing assessments. Pretty much everything. I was just wondering if anyone has good ideas on how to organize a clinical binder and or if anyone has a cardiac med chart that they would like to share! Hope I get some feeback. thanks guys!..My clinical instructor is TOUGH. I've already been told I suck at reporting lol.
  2. Hello all... I am in an accelerated BSN program and currently in my second level of the program. Here is the issue: starting my second clinical rotation and now I will be on the PCU floor with a very tough clinical instructor. I was babied the first semester, my first clinical instructor was very nice and patient. This instructor is very strict, we do not get a patient we are assigned to a nurse and follow her with her 4 patients. We then get to pick one patient in which we give our instructor report about, including meds. She is VERY strict with Meds and only gives us 15 min to look up the information about the patient. I'm worried I won't know an answer to her questions about the pt/meds, I am also very intimidated by her. Anyone have any advice as to how to answer her if I am not sure of an answer. Anyone have experience on a PCU floor, what will I see, what drugs will i expect to see, I need all the preparation I can get. Hoping someone can help!
  3. I'm having difficulty putting my thoughts into words. I have an assignment on writing a nursing note for a skin lesion. If anyone can help me use the right words for documentation I would really appreciate it. Also, if anyone knows any good examples or websites that have examples of how to chart/write nursing notes that would be great too! I really want to learn how to document correctly! Here is what I know: location of the lesion is on the right antebrachial,the color of the lesion is brown, the size is >1cm (3 cms), it has a symetrical shape, there is discrete distribution, there is no exudate, and i believe it is called a patch (it is a flat birthmark). If anyone can help me make that sound more like a nurses note, I'd really appreciate it.
