  1. Hello everyone, i have a care plan due .soon and i needyour help. I have a pt of 78 yrs old w no family. She had a cva and now has a riht sided hemiplegia and expressive aphasia and right hemianopsia. She is on diuretic and beta blocker. She is on a nasal canunula however she does not have breathing impairment. She is not able to walk independently, walk up steps, rise from a chair without assistance. She will be transfererd.to a SNF today. Right now i have her as my pt for my shift and need a.care plane for her (short term and long term). My NANDA is 1) her actual diagnoses: impaired mobility r/t right sided paralysis secondary to CVA2) her potential one is pt unable to hold (control) urine. Potentialy at risk for impaired skin integrity. R/t urinary incontinence. Secondary to.CVA. Implement/intervention: i want to be.able to do ROM on my patient assisting her wholly and possible partly when it is time of discharge. As for urinary incontinence help her with it by kegel exercise. I am not quite sure what is right. I hope someone can be so kindly to help me figure this this out...please help.... :c
