Sorry for the delayed reply I've been going through a lot of health issues and haven't had time to get back. Thank you everyone for all your input. I actually put in my resignation before any of this occured and am on my last week. This issue being one of the many reason I no longer feel comfortable working here. I feel better knowing I was not alone in thinking this was a terrible idea. Tomorrow I will be forwarding her the list of patients that can't be completed notifying her that I will not be documenting for anyone else when I was not there to ensure that it was given. I plan on giving the suggestion of having the providers-who had ordered the vaccines, providing they are sure of their documentation that do not mention vaccines rejected, not given ect ect record that the vaccines were given as they are the only other person who can actually attest to the patients receiving the vacccines. To everyone that has given a senario-those exact senario's are exactly why I was not comfortable with it. I honestly have no idea why things were not documented-it could have been a simple mistake where the MA for got or accidently missed the documentation. Or it could have been not documented on purpose due to it not actually having been given. Sigh...thanks again everyone for your words of wisdom and support. :)

Because we do daily inventory I can determine the lot and exp that was available that day. The things I can't determine for a fact is the person that gave the vaccine and if the vaccine was given for sure. My DON seems to be set that I will be doing this. Her email to me pretty much stated that I needed to problem solve and get it figured out and get it done.

Sigh...I'm being asked by my DON to document immunizations given by MAs who are no longer with our company and who failed to document and submit them. I am completly uncomfortable with this. I was told today that if I don't know if it was given I need to look at the doctors note to determine if it was given, if I'm still not satisfied I need to contact the parents of the patients and ask if the patient received the vaccinations. She told me all I have to do is document that it was given and also document what ever MA I presume gave it, gave it. The rational is that the patient needs to have a correct MR so that they don't end up having to get the same immunization. The problem is I can't guarentee what MA actually gave the vaccination as there is no documentation to tell me-I can tell who roomed the patient but I can't tell if another MA jumped in and gave the immunization and I can't be 100% sure the immunization was actually given in the first place. A majority of the patients will have a form scanned in the chart with an order/document that states that at that visit with those imz ordered they are now up to date on their IMZs-but again there is no signature by anyone that actually would be giving the vaccines, only the doctors signature at the bottom that should signafy that they ordered those vaccines and they were given. I can contact the parents but so many parents can't tell you how many immunizations their child got on any given day (and I'm looking at a 5 month time span) let alone what they were. Does anyone have any advice on what to do or resources that I can go to? Thank you!

Sorry its California's Board of vocational nursing and psychiatric technicians. That's what I was thinking of doing. I just don't even know where to look for a lawyer for nursing issues.

I was fired from my first nursing job 2 years ago. I had only worked for 3.5 months. We charted pt's I&O's in a computer system but the CNA's were also suppose to write them down for certain patients in a log book at the end of the shift. The state had come in and because of this the DON had stated that under no circumstance was the hand written log book to be left blank. One day both of my CNA's had left and not written anything in the log book. I was told by the girl that helped trained me that I needed to write down in the log book estimates based on what I'd seen through my shift. So I wrote down what I knew. If I gave a patient 2 glasses of water I wrote that down as his Intake and if I had checked on him and noted that he had 500cc I wrote that down or if I had helped them to the bathroom I wrote that down. I worked the night shift and I stayed late-which wasn't even allowed, they were very addamate that under no circumstance were we to get any overtime, to go through my notes and recall what I had seen and done to write it down-because I had no other option, the computer system that logs the I&O clears after each shift. My supervisor came in and stated that I was no longer suppose to be there and to give her the notes from the CNA's so she could finish documenting for me. I told her that my CNA's had left without notify me or leaving anything. She then asked how I was documenting and I told her I was only documenting what I had observed. She became furious and yelled about the state being in the facility and that I should have called my CNA's to come back in (I did not have any of their contact information and admin did not open for 2 hours after my shift). I told her that multiple nurses that she had put in charge of training me had told me to write down what I knew if I had nothing else to go on. The DON had me leave and then called me later that day and put me on supsension pending investigation. I was then called in and fired. I asked if an investigation had occured and asked what they had found and if they also discipline the others who were instructing others to do so and all I was told was that no investigation was needed as they had my words to go of, because what I wrote down could have been incorrect because I didn't have the documentation from my CNA's. That was 2 years ago. I just got a voice message from a special investigator the BVNPT department of consumer affairs today stating they were notified of my termination and were wanting to close it out and they need to ask me what happened and get my side of the story so they can close it out. I'm just looking for recommendations on what to do. Or things to avoid saying. Any advise would be greatly appreciated
