ummm actually i doknow how to do care plans im just scared and dont want to mess anything up. not evryschool has there students do care plans the same so before u judge just remeber when u went through a hard time in school and just needed a littile reassurance. if u never struggled kudos to you. however i have made it through this far because im smart.

I'm care planning an 89yro pt admittied for a tia and seconday dx of a uti. I'm havein troublr coming up with nursing dx so far I have: Ineffective peripheral tissue per r/t atherscerlotic occlusion/processes Risk for infection r/t ???? High risk for falls r/t ???? Knowledege deficit r/t I think current health status/ disease process. But idk??? Impaied short term memory r/t???? Self care deficit bathing dressing/grooming/toleiting r/t???? Please help me even the slighest bit can make a huge difference. If I don't pass this I fail nursing school and I only have 9 weeks left!
