Dimples the health/physical form is on the certified website on the drop down menu that says health/physical form. You have to print it off and her it signed. Hope this helps.

I had to have all of the same paperwork last year when I did a travel nursing assignment so getting all the documents together was a breeze. You could always get shot records from your local health department, they may or may not have an up to date record but it's worth a shot.

Anyone else in GA get into the BSN-DNP FNP track?

I emailed the academic advisor for the DNP program. She actually emailed me my acceptance letter and said that the letters are in the mail. I'm not sure what the deal is with these acceptance letters but I'm wondering why many of us have not received them yet. Did you all use this email to inquire about the acceptance letters : [email protected]? Hope this helps.

I got the same email just an hour so go I havent received a letter in the mail yet either. I hope this means we got in!!!!!!!

I did not receive anything today. Sorry for the late response, I was at work. What specialty program did you all apply to? I applied to FNP. I received this email yesterday: Dear Applicant, We have mailed most decision letters out to DNP applicants today; however, decision letters to those of you who applied for the Advanced Emergency program will not be mailed out until Monday, May 12. We appreciate your patience. No response to this email is necessary. Thank you.

Hello Everyone!! I also applied for USA's BSN-DNP FNP program for fall 2014.Late November, I received my email stating they received my application and that they wouldn't make any decisions until after the March 1st deadline. I guess we won't find out till late April (that's when fall 2013 applicants found out). I am keeping my fingers crossed. Good Luck to everyone!
