I received this letter yesterday and this is the stated , I'm so confused coz I already sent my requesting which is pages 13 17 and 22 form when I was in the Philippines last April and almost 1 month. An initial evaluation of your application for your examination indicatesthe following item to complete your file or indicates information pertaining to your file Please provide all of the following documents: - Have your nursing school submit a copy of letter(s) from CHED approving the nursing program your complete. - Class schedule for the nursing program completed ( RLE is not a class schedule) . - Please have your nursing school submit a academic school calendar, which reflect exact dates each semester all holidays and every school break begins and ends. - A copy of registration/ enrollment form- generally given at time of registration. - A copy of your student handbook. - Please have your nursing school verify that the hours reflected on the educational breakdown form do not include any health care l/ll hours .if so , they will need to submit the information without the health care l/ll hours reflected , this information must come directly to the board in writing from your nursing school. - A copy of your US SSN. - A copy of your entire visa/ or passport including blank pages for entire timeframe in nursing school. This information will able the board to determine if the nursing program you completed meets all educational requirements necessary to qualify you for the examination. If we do not have receive the item listed above within one year from the date of this letter your application considered abandoned and your file will be destroy.
