For the employment verification i can't help because i'm a new nurse and i have no experience. But for the education forms I'm pretty sure they are required and that a transcript won't do. I live near my school so i was able to go up there and they filled out the forms while i waited. If you live near i would call and see if they will do the same. If not unfortunately you will need to send them the forms and get them back. I would call them and let them know that you sent in the forms so that they can be looking for them and they don't get buried under other paperwork. I don't know how organized you are but if your like me ( a total mess) I would ask if they are willing to sign more than one copy. If you have any other questions i will try my best to answer. I live in IL, but went to school in St.Louis and i got my license for MO thinking that i would find a job right away. Well i graduated in May and still haven't found a job so it's time to widen my prospects and want to get my IL license as well. I figure as a new grad if i have to take a job i don't really want it might as well be in IL so i don't have to deal with crossing the bridge and traffic everyday. I've looked at the application and for the most part i've got it filled out. There are just a few questions that are stopping me from finishing and turning in the application. (I know my best bet at answering these questions are to get in contact with the IL board but every time i call i get the automated message saying that they are busy and to try again later. I've been working on this application for a couple months now and really would love to get it done so i can finally get a job) My questions; 1. Can apply for a temporary license and endorsement at the same time? So can i turn in one application and just make sure i have everything else i need for each thing and be ok? Or do i need to send them in separately? 2.I don't currently have a job and haven't worked as a nurse so do i need to include the verification of employment/experience (VE) form? 3. For the CT-NUR form since I'm licensed in MO and they use nursys I don't need to complete this form right? I just need to go to the site and follow the instructions there? 4. Do i need to fill out the Health Care Workers Charged with or Convicted of Criminal Acts(CCA)? I feel like this answer is no because i haven't been charged or convicted but just want to make sure. 5. For the background check is all i have to do is get finger printed and they will do the back ground check based on that? Probably another duh question but just want to make sure. I appreciate any help, it's been a nice break but i'm ready to finally get this finished and so i can start applying.

Hi, hopefully someone can help me. My patient had pneumonia and c.diff was pretty skinny, had a dobhoff in and a central line (although i'm not sure the last one matters) I was thinking that my main diagnosis would be altered nutrition, but looking in all my books I can only find altered nutrion; more or less body requirements. I would choose the one for less body requirements but one of my books said that this diagnosis wasn't for people who were NPO (which they were) . So now I'm confused on what diagnosis i can use that deals with altered nutrition. Also for the as manifested by's i've got diarrhea, placement of dobhoff, and weight loss. I know there based off what is in the chart but are there other types of things that i should include in that list. Pretty confused and my books are only making it worse. Thanks for any help.
