Need Help With My Clinical Simulation
I'm a first year student and need a little assistance with this clinical simulation project that I'm supposed to do. There are 3 others in my group and I just had to post this to make sure we did it right. I'm almost positive theres stuff we left out. Heres what we have to do. Theres 4 of us all together and we all have specific roles. One is going to be the nurse practicioner (me), another the nursing studnent, another the assigned nurse, and the other the family member of the patient. Our simulation is fairly straight forward and easy. A 45 year old female is assisted into the ED by wheelchair. She suffered a fall while walking up an icy staircase and injured her left lower leg. She unable to bear weight. Past medical history is insignificant and she takes no medications and has no allergies. She's admitted and the nurse practcianer orders X-rays, ice to left leg, and 5 mg IM of morphine. Were supposed to clinically simulate this simulation and what we would do. We told our teacher that we would get the xrays bakc and there was a slight tibia fracture. She said that was fine. How would the ED handle this patient? Specifically what would the nurse and NP being doing? Were going to have the nurse delegate vitals to to the nursing studnet since her scope of practice is limited. Were also goign to have the nurse vocalize her assessment (this will be a focused assessment for example) and teach/explain why she is doing what she is doing. Another thing we had trouble with was wondering if the pt would be casted or not in the ED. Its a slight fx but does a orthopedist still need to be consulted? Please help us...we need these points :). -Future nurse

I want Nursing Central for my ipad for school but their cost is pretty steep to say the least. 160 dollars for a year? Seriously? Does this thing cook me breakfast, lunch, and dinner, as well? Is it worth it? What is the cost after the first yr to renew? Anyway I can get a discount???Any and all advice/experience is appreciated.

Thank you everybody, again. I'm kind of at a laughing point in my process now. All your experiences are exactly what I needed to get to the laughing stage. I know I'll be faced with this again. Learning through experience has always given me the most growth. Lesson learned.

I work as a PCT at an ICU and just currently started nursing school. Last Thursday towards the end of shift I was waved into a room by a doctor. I rarely talk to the doctors at my job. It's not personal or anything like that. They know they don't need to confer with me and I know I don't need to confer with them. This was a peculiar set of events and I was curious to say the least. As soon as I arrived in patients room I was immediately talk to in a way I have not been talked to in a long long time. I was immediately demeaned as she badgered me about how nobody on this unit follows the rules and the patient should not be sitting in a recliner in from of thousands dollars worth of equipment (she was referring to the bedside computer). I was calm and asked if she would like me to move her up a little so she could use it. She immediately barked back 'yes' and that would be needing a chair. I walked over past the cabinet and told her there is usually a chair hanging on the wall in every room but this time there wasn't and she barked back again. I wish I could paint the picture of how she talked to me because I'm not doing her any justice on this post. What's comical is I've never seen a doctor (Ever!) on this unit chart by the bedside. The hospital is brand new and they were given their own private office full of computers where they can chart but they instead use the ones the techs and nurses need. Her high and mightly attitude blew me away. The daughter of the patient was even the one who put her in the recliner but that didn't matter and that isn't the point. Where the patient was, was an ideal spot because it allowed her to be right in front of her television. In my head while I was in the room I was thinking..."Sorry Massa, I don't know how the chair got in front of the computer massa, how can I help Massa?" My dad was a doctor and didn't treat people like that. You treat everyone equally in my book. Your profession is a huge part of you but it is not who are. Everyone from every aspect of life should be treated exactly as you would treat a doctor, the pope, or Jesus Christ reincarnated. I can't seem to let this thing go. It's been three days and I still have not let go of this. I could use some advice.

What are units when referenced for medications?
Sorry, this is probably a stupid question but I'm tired of always wondering as a PCT how much of a drug nurses are talking about when referring to drug measurements in "units." Is it different amounts with different drugs? For example, 1 unit of insulin is how much? Thanks for the help. I start nursing school in 2 days. Figure it is time I know this because it has bothered me forever. Thanks!

should i buy these two books or just get nursing central on my ipad?
Ok, so i start nursing school in a couple days and I have been busy buying all these many many books. My question revolves around two books: Lippincotts drug guide and Tabers cyclopedic medical dictionary. Together these two books cost like 45 to 50 bucks. But then there is this wonderful nursing/nursing student app i keep hearing about: Nursing Central (which includes a drug guide and the exact Taber reference plus laboratory info and many other very useful things).However this app is like 170 for a yr subscription...pretty stiff. Anyways...can i please get some advice??? What would you go with? Any experiences to back something up? I'm leaning toward getting the app. but just don't know.

So I'm taking pharmacology this qrt. Right now I'm in my school's library kind of bummed out. I'm reading the chapter on drugs to treat hypertension. Theres all these new things being introduced to me that are kind of freaking me out a little. All the various drugs used to treat hypertension. Beta blockers, diruetics, calcium channel blockers, ACE inhibitors, Alpha-1 Adrenergic blocking agents, etc. etc. etc. How in the hell am I supposed to know the drugs for these different types of hypertensive medication categories?!!! ***!!!

I'm almost finished with prereqs to get into nursing school. I have a laptop but am drawn to the idea of owning an ipad 2 that i can carry around with ease. Should I buy an ipad 2? Is there any experiences out there that can help me make this decision? -C

Aids giving me trouble-what to do?
I'm an aid at the moment and currently in nursing school. What has been posted so far is on point. I've only been an aide now for a little over 5 months. I worked in LTC for 3 and now am working in an ICU. ANYWAYS... It is time to pull rank. I've been in this work for 5 months and all ready I'm observing ridiculus politics that are frankly just meaningless and basically fall depth to just the idiosyncrasy and fault of being human. I want to be a nurse some day and the **** I see passing between fellow aides gets on my last nerves. The best advice I can give is to lay aside bipartisanship and lay reality down. Talk to the DON and lay out time for an intervention. Sorry, but theres too many people out there who want a job that can do a better job at treating people in the twilight of their lives.

Hello, To be quick I have currently worked for a long term care facility for about a month (nurse aide) part-time. I have now decided to take on another part-time Tech position working for a hospital in a step down ICU. I have not told my previous employer about this yet and it is starting to gnaw at me. My plan is to work both jobs for the Summer and then drop the job at the nursing home so I can maintain my nursing school commitment. I want to do this in the most professional/humble way I can and I need advice on how to do this. I think I would be acting with integrity by telling her I'm accepting this position but I don't think it would be very smart of me to tell her I'm planning on quitting after the Summer. Please help. This is really bothering me. Some people might think this is really a small matter because I've only been there a month but I want my career in health care to be one of complete honesty and integrity. I've been given this one life and I'd like to be remembered down the road for someone who handled delicate situations with some finesse. Please give me your thoughts. Thanks.

Ok, I want to make this short. Yesterday I had to get a physical for an aide position I'm taking at a step down ICU. I'm currently in a community college a couple classes short of finishing my prereqs for the nursing program. I'm on a undetermined waiting list and getting very impatient. I graduated in 2009 with a Bachelor of Arts majoring in English. On to the meat of the story... I'm getting looked over by the doctor during the physical and telling her my story. I told her how my dad was a doctor and just my luck she knew who he was. Her response to me becoming a nurse "Why don't you become a doctor?" "Didn't you take anything throughout college related to science?" "Where do you go to school? Your at a community college? Oh, honey you're waisting your time. You have a bachelor already, you need to get your bachelor if your going into nursing. Bottom line your waisting your time at a community college." I was floored. She was so cut throat and I really feel like she knew what she was talking about. What are your opinions? Do i need to transfer and just to nursing at a university? My understanding is I'm going to be spending the same amount of time to get my BSN than I would for the associates. I almost feel dense asking this since I juts posted the obvious in the last sentance above. I just know how much cheaper it will be at the community college. Am I wrong? I feel stupid to even post this. Believe me...that feeling is rare. ??? -Chris

Hi I'm a nursing student working two jobs as a tech on ICU and aide at nursing home. I'm about a month into the jobs and being up on my feet 24/7 is already taking an amazing toll on my feet. I wake up in the morning and its really painful just to walk around. I'm off today and just walking around everywhere is ridiculous with the pain. Whats the inside secret (God, please let there be one) on how best to learn how to deal with this or avoid. Are there particular shoes out there that help. If so, please let me in on the brand and EXACT model shoe. I'm dieing over here and I feel like a total wuss for posting this. I'm a guy too so my ego is going out window when I'm talking about something like this (booo hooo don't cry for me Argentina!). :crying2: Any and all help is appreciated! -Chris
