  1. I'm currently working on a nursing care plan for ineffective protection r/t abdominal wound secondary to MRSA. My as evidenced by is as follows: Temp 99.1, purulent drainage from wound, redness& warmth around wound, fatigue, decreased RBC 3.2, decreased hemoglobin 9.7, decreased hematocrit 28.8, increased WBC 11.9. What I'm having a problem with is the interventions. I currently have to come up with 6 interventions (3dependent, 3 independent). I only have 4! Assess pt vitals, drainage, etc., administer antibiotics as ordered, teach patient sterile technique, & notify physician of increased drainage, warmth, increased WBC. Could you please suggest more interventions? Thanks so much.
