ok my patient is a 48 year old female bedridden and chair bound all the time she needs assistance to ambulate and for all self care. she has anoxic encephalopathy with brain damage. i need to do a 3 page care plan using 3 different nursing diagnosis so far this is what i came up with for diagnoses impaired physical mobility r/t muscle weakness and disease process self care deficit: dressing and grooming r/t cognitive impairment chronic confusion r/t impaired decision making ineffective coping r/t impaired information processing noncompliance with nursing staff r/t behavior problem due to mental decline impaired verbal communication r/t aphasia-speech deficit risk for falls r/t muscle weakness risk for impaired skin integrity r/t bedridden/chairbound so now i am trying to choose the top three prioriy dx and having trouble. i understand they need to be in order starting with the abc's so thats why i am having trouble because my patient has no breathing issues or heart problems. also her skin is intact w/o signs of ulcers or breakdown. what 3 do you think are priority nursing diagnoses? thanks i appreciate your ideas and responses:redbeathe [color=#888888]
