  1. Yeah the fragmin was already being held. The doctor was aware and a hematologist was on the case for that and other reasons. And I gave my explaination for what I thought the doctor was holding it. And she acted like I was stupid because pt/inr "is not related to fragmin." And I understand that, yet the pt was at risk for bleeding.I just told her she can ask the doctor that day if she wanted to know what. But I was sure he would keep it held. I just left angry and really wanted to give her a piece of my mind because this is not the first time she has acted like that. I just figured "common sense! Very high pt/inr, then hold all anti-coags and anti-plts and give VI." Thanks for the replies I just fu

  3. I have been a nurse for almost a year now and want to become a home health care nurse. I have been working in med surg since I graduated and three years before as a student nurse and physical therap assistant. I think I tould really enjoy home health care and someday case management. Could someone tell me what they think of home health care? Is is best to work for a hospitals hhc or agency? If I work hhc will I lose some skills? I want to get my masters in 2011, will this hinder my experience for school? If you have been in hhc, is it ever scary to be going to someones home alone? Do you have any other advice or comments?
  4. OK, I graduated from nursing school a year ago and gave worked as a nurse on a Med surgery floor since. And I feel like I still to not know enough about pt/inr and platelet counts. Can anyone tell me if a patient has an elevated pt/inr of 24 secs and inr 4.2. Wouldn't it be a good idea for the doctor to hold fragmin? The plt count was OK at 277. I mean even though pt/inr is r/t coumadin doesn't it make sense to hold the fragmin for risk of bleeding? An older nurse i gave report to thought I was an idiot for saying that because they Are not related, yet they both have a risk of bleeding.
