  1. Yeah, I wouldnt worry too much about that. Its your first mistake, and thankfully no harm was done. Ive seen mistakes much worse, and harmless mistakes. I accidentally gave a patient colace when it wasnt ordered. No harm done, and in the end the doctor ended up signing an order of colace for the patient. No corrective action was taken, my RN just told me to more alert, and I was. I've seen Lovenox given to the wrong patient and in the arm. I've seen wrong IV fluids hung. people make mistakes all the time. Really, if your going to beat yourself up about it, beat yourself up about serious mistakes, like narcotics. My RN gave scheduled Demerol to her patient, but forgot to sign off on it, and during the change of shift, it wasn't mentioned, so the following RN gave the patient another dose of Demerol within 10 mins. It wasn't caught until two hours later when the previous RN came back in and said she forgot to sign off the demerol. An incident report was filed, both nurses were given a counseling statement, and in services on medication error had to be given by them. Another RN of mine, gave a patient an overdose of morphine because the doctor was new and worse the order wrong.
  2. Okay, so I am new to thread and discussion, so HI everyone. I need help. I am LPN for the US Army. The Army trained me in their PN course, and I got my license by the state of Texas as do all other Army LPNs. Now, I've already taken the NCLEX for the state of Texas during my Christmas leave in Indiana. This was about two years ago. I'm a reservist now, drilling in Florida and trying to get a NY LPN license by endorsement. But It says that I need to fill out Form 2 for Certification of Professional Education, and then it states that all LPNs by the US military must fill out Form 2AF. Which is "Certification of US Armed Forces Education for LPN". Both forms have to be submitted by the certifying facility. What I don't understand, and need help with, is this. Do I have to fill out BOTH Forms? It just seems redundant to me, considering that I didn't receive any other nursing licensure/certification from a civilian school. And my second question is..where do I send the form?? Texas AMEDD is the school headquarters so to speak, and everyone does Phase I of LPN school there, but after that we get sent to 5 different hospitals for Phase II, I graduated the PN course in Georgia, underneath the name of Texas. Or can I just have my reserve unit fill out Form since they are the military unit in charge of me... I am really confused. And completely overwhelmed by all this paperwork, and not knowing where it all goes. If anyone can help me please...
