  1. I am 48 and going to finish RN-AD in May, rcvd LPN in July. Have son in JH, son in college, very supportive spouse. Don't ever doubt what you are doing. we rock! difficult task? they ask us. difficult patient in clinical? they give them to us. always complimented on how positive we are. There are several 50ish students in my class. Our motto, "What's the worst that can happen? they can't eat us!" This we say before skills labs, tests, and clinical. We ask the questions the young ones are afraid to ask. We are better at negotiating with the instructors and everything else. We get more respect when we do clinicals because we are closer in age to a lot who work there and a lot of people in the hospital. Don't ever give up!

Impaired physical mobility alteration in nutrition: less than body requirements (she will need more protein to heal fractures) probably not going to eat as well due to depression and what she has been through Activity intolerance. You can google free care plans and come up with more diagnoses and goals and interventions. I am LPN enrolled in RN ADN program and use some great nursing care plan books also.
  3. I am not sure how many dx you need but, if she is going to go on dialysis does she have the knowledge to deal with this huge chg in her life? Deficient knowledge R/T? Is she following the strict renal diet regimen? Most dialysis pts spend several hrs a day, 3 days a week on machine. Yes, they can have big problem after dialysis with dehydration. Taylor the plan to pts situation and needs.
  4. Did you choose this diagnosis? anyway, I'm LPN to RN student. Heavy on the care plans. Is the pt going home or LTC facility? Is loss of sensation R/T diabetes? are they mobile? is loss of sensation in feet also? if so you could do some of same teaching and interventions for diabetic foot care. well fitting shoes, inspect daily for cuts, etc., do they live alone or have caregiver. really need to taylor plan to pt situation. there are good care plan info in decent med surg books and there are care plan books as well and some websites.
