13 pound infant...thank goodness it was c-section!
2 wks ago we had a 11lb 8oz baby girl born via c-section. The top of her head was the shape of mom's pelvic outlet. She was the ugliest little thing I'd even seen in my life! She did get cuter over the next few days. 2 days ago had a 10lb 7oz baby boy born lady partslly. Mom did great, did get an epis but said it was a much easier delivery than her 8 pounder. Go figure. Gotta love epidurals for that reason!

Crash Sections
Our department just set up protocol for this type of c-section. We do not have anesthesia in house after 5. We have just been lucky by the grace of god! Earlier this fall we had a uterine rupture walk in around 9 pm. Luckily anesthesia was in house for another c/s for failure to progess. This women was actually on the table..they hadn't cut yet so they moved her off and got the other woman in there. From the time of admission to delivery was 8 min. The baby had a ph of 6.7 apgars of 0/3/6. She is 7 months old and doing okay. About 3 months behind.

Slow February?
We've had a slow Jan/Feb. It looks as if it will pick up in March. I've done a lot of floating out to M/S floor. We went 9 days in Jan without a delivery! I think we are working on 5 days now. Small hospital, about 300/yr. We just had some change with an OB leaving, new one coming. Our manager thinks it's really going to pick up. We hope!

Old School
Shaved?? Don't most come in "prepped"already?!?! When I was orienting a few months ago, one of the "older nurses" says to a patient..okay enough, what's up with shaving the pubic hair off..I haven't seen one with hair in months. I about died laughing. The girls reply...my boyfriend likes it that way.

Intubation
I work in a small hospital birth center. First of all our Mec deliveries are required to have a NRP certified physician, actually any doc that see's a baby must be NRP certified. All of our nurses are NRP trained also. What if you had a case gone bad, gone to court and no one was certified. That wouldn't look good. As for intubation if needed the RT or Doc does it. Hope that helps some

Report
We used to give group report and too much time was wasted on nothing. A few seasoned nurses would question other nurses interventions etc. A lot of wasted time. So our manager decided to go to tape report and it goes much quicker! We have a check list to go by so we don't miss anything. I tend to stick to exception reporting since there is a sheet that gives you the "stats" of the patient. Hope that helps

Cord Prolapse
To add to your question about cord prolapse, I was just studying for a perinatal core class. You do want to wrap the cord with gauze and saline to prevent the cord from spasming. This will also help optimize fetal oxygenation. Hope that helps
