  1. I used to be licensed as an RN in Colorado. I have a BSN and almost 20 years experience as a nurse. But I am also an addict. I have been clean and sober for over two years and have no felonies. In 1995, I self-reported to the Board of nursing and entered into the diversion program that was in place then. In 1999, I quit my job and stopped nursing. I relapsed and did not contact the board. My license was revoked in 2000 because I had not been in contact with the board, which is a violation. I have not worked in the field since 1997. I have not been ready to try again until now. I don't know where to start to figure this out in Colorado. Do I contact the Nurse Health Program, or the State Board of Nursing directly? Any ideas? Thank you
