I resigned from my last position almost six months ago. I didn't like where I was working a whole lot. Job satisfaction was overall pretty low with everyone, the nursing stations and med rooms were always a total mess even after I cleaned them by myself, half of the techs were nearly useless and one even hid and slept in empty rooms, and I wasn't very well respected as a nurse or even a person by some of the people there. I have a ADN and I am in the RN-BSN-MSN program now. I have a year of tele experience and multiple certifications...TNCC, PALS, ACLS. I also had a critical care preceptorship in school and I am a member of the ANCC. It's been almost 6 months and no job offers. I have applied for every position, hospital, and department that I can think of...home health, outpatient, OR, med-surg, cardiac, ICU, ER, step down units, GI lab, even tech positions. I have had four interviews, two for ER and two for ICU, just no offers. I also kind of need something that will work with school. I fear that the longer I go the tougher it will be to get a job. Any words of wisdom? By the way, I REALLY would prefer critical care because I need one to two years of critical care before I can start my masters program. Thanks!

I am an american nurse needing to move to brasil for personal reasons. I am an RN with an ASN degree. What steps do I need to take to practice in brasil? I am assuming that there is a nursing test of some kind? If so, what is it called and what organization is in charge of issuing the license? Is my ASN good enough or will I absolutely have to have a BSN? How do I need to prove fluency in Portuguese? Any other advice would be most helpful. Obrigado!

ghost116, I am actually trying to find a way to do the opposite. I live in Georgia and I am trying to find a way to move to Sãp Paulo for personal reasons. I am an RN with an ASN and most of the undergrad work done for a BSN except the nursing courses. My portuguese isn't exactly terrific but I am learning quickly. I think we can help each other. There is probably a ton of legal stuff I know nothing about. Do you know how I get a license to practice nursing in brazil? Is there a test I have to take? What association is in charge of issuing the nursing license in Brasil? Will I need a BSN or is my ASN good enough? Almost all of the nurses at my hospital are immigrants so I can ask around if you have any questions. Please help me!!!!!

From Harrison's principles of internal medicine... "In pulsus paradoxus, the decrease in systolic arterial pressure that normally accompanies the reduction in arterial pulse amplitude during inspiration is accentuated. In patient's with pericardial tamponade, airway obstruction, or superior vena cava obstruction, the decrease in arterial systolic pressure frequently exceeds the normal decrease of 10 mmHg and the peripheral pulse may disappear completely with inspiration." so basically the systolic pressure/pulse amplitude decreases more than usual on inspiration and can be due to a life threatening condition. I suppose you could use an arterial line waveform if you want to be precise or just palpate, but i think the biggest thing to remember is to look at the patient. Since this can be caused by life threatening problems they will or will eventually show other signs and symptoms that will point you in the right direction. Hope this helps...
