thank you guys! I'm very excited that this is it! The pt is a 11month old male who was brought in a month ago because he had a fever, they thought it was pneumonia, they did blood work up and found that he had leukemia. as of now he is stable..thats why they were risk for nursing diagnosis. what do you think? he is being housed as of now at the hospital until he can go elsewhere for a bone marrow transplant.

last nursing care plan ever!
so..this is my last care plan ever and I'm a little stumped for nursing diagnosis. I have a 11mo who was diagnosed about a month ago with ALL. For these care plans we have to write psychosocial nursing diagnosis based on their developmental task (i.e. trust v mistrust). Our instructor wants them to be PATIENT focused, not parent focused so, it makes it a little more difficult. With this said she does not like impaired parent/infant attachment as this is more parent focused than infant. The nursing diagnosis that I have come up with for this particular patient are 1.) Risk for infection r/t immunosuppressed state secondary to leukemia and side effects of chemotherapy 2.) Risk for injury r/t thrombocytopenia 3.) Risk for disorganized infant behavior r/t prolonged hospitalization, unfamiliar environment, stranger anxiety and painful procedures. Let me know what you guys think or if you have any suggestions. I hate that they are all risk for but, his breathing pattern is ok, airway clearance is fine, bp/circulation is good, fluid volume status good, nutritional status good, his mucous membranes are good thus far through his induction phase of chemotherapy.

need help writing psychosocial nursing dx based on developmental tasks
Hi...I'm desperately hoping that someone can help me out because I'm not grasping the concept of nursing dx based on developmental tasks...(i.e. trust v mistrust etc). Also, these obviously have to be patient focused NOT parent related. The parents are obviously a part of our interventions, but the nursing dx cannot be r/t to them. Anywho.. I've got an unfortunate pt who is 8 years old (her developmental task is industry v inferiority) she is developmentally behind so she may be between stages however, we're supposed to focus on where they should be. her HPI is as follows: she has a h/o seizure disorder, on 1/12/09 she was found with clenched hands, moaning in bed, +salivation, + tongue biting, no loss of bowel/bladder..she was taken to hospital..etc prior to event pt had been developing her gross motor skills appropriately but had delay in visual motor and fine motor dexterity (age equiv 4-5 years) has social perceptive issues-does not understand sarcasm & if people are laughing at something she doesn't understand she gets upset & believes they are laughing at her. possible psychosocial nursing dx based on industry v inferiority that I have come up with are as follows: 1.) Risk for chronic low self esteem r/t social isolation 2.) Impaired social interactions r/t anxiety amb pt gets upset if laughing around her and she does not understand what the laughter is about 3.) Risk for Social isolation r/t prolonged hospitalization 4.) Risk for lonlieness r/t social isolation from peers secondary to prolonged hospitalization and impaired social interactions Let me know what you guys think!! PLEASE HELP! I'm 2 months from graduation!

HELP! Concept map...DUE MONDAY!
Ok, so i have a "concept map" due on monday for one aspect of nursing care for a patient. The directions state as follows: "choose as the 'center' one important aspect of nursing care specific for your patient. Create your map surrounding this concept incorporating developmental aspects appropriate for the age of your patient. Do not use your patient's medical diagnosis or re-create your entire nursing care plan for the child into this format." I have chosen overcoming oral aversion in 6mo pt with NGT & Trach. I'm not exactly sure how to begin to approach this... I'm not sure what the instructions mean for "developmental aspects" either...I get the whole Trust v. mistrust is the developmental task according to Erikson but, I'm very confused on how to go about doing this. Any help? PLEASE!! Maybe a few ideas to get past oral aversions for those of you who may have experience with this would be great..or maybe just some ideas on how to get started with this assignment would be greatly appreciated!! Thanks sooooo much!!!

Peds terrifies me
I'm in my last semester of nursing school and am doing peds and ob this semester. I have been looking foward to this semester but my instructor is so picky about our care plans. We have to have 3 nursing dx one being a psychosocial dx. I've already taken Psych nursing, but for some reason adding peds to the mixture of psychosocial dx has thrown me totally off. I'm freaking out because I'm just ready to be done with nursing school! My first patient at the peds rehab center was: 6month old term infant with dx of omphalocele, the parents are not able to be there due to financial reasons. The infant is accomplishng the skills of a 6 month old. she is trach/vent dependent, ngt dependent. My #1 nursing d/x was risk for ineffective airway clearance...#2 was risk for infection (which my instructor stated was no longer an issue with her omphalocele b/c it had almost completely epitheliazed) #3 was readiness for enhanced communication r/t the infant trying to mimic sounds made with mouth, smiling at self in mirror etc. (which this is apparently not a psychosocial nursing dx) i was wondering if anyone could help me out and let me know what I did wrong. Thanks so much! I'm sure I'll be posting more throughout the semester because Peds nursing dx/interventions are not my strong point and I just really want to finish nursing school!!! PLEASE HELP!!!
