  1. Her delusion was not good for those around her before she was brought to the institution.. She chased with a "bolo" those who brought her in.. That's why I wanted to remove her delusion about people who want to do bad things to her (as she perceived).. Or maybe just assure her that no harm will be done to her..

Thank you for that detailed reply..:) ..when i was interviewing her, she did not tell me that she can hear voices and she was very attentive during our conversation.. she told me that she was brought there for protection from the JFK followers/bad CIA.. the most i got to know was up to that $5,000 issue.. ..with regards to her delusional family, i was not able to focus on it since she believed that they were real.. by computing for the ages she told me, i was able to verify that it was really a delusion.. the charts offered so little about the patients and we were not allowed to interview relatives..
  3. Thank you ma'am Whispera for that lovely comment.. I just thought of rendering her the care that I should have given to my late grandmother.. ..Yes, we call them by names.. We practice it in the clinical settings.. I just use the term client when I make my case study, in place of the client's initials to avoid monotony..:) ..in addition, not all clients are ill, that's why I prefer to use client instead of patient (stereotype for ill individuals) since they came to avail the health care services, may it be for wellness or treatment..:)

I hope this would be the last for now about the tag for the individual seeking health care services.. ..being primarily affiliated with a tertiary hospital, we were trained to use clients because they are availing the health care services.. ..sorry if I'm ending this topic now, just want to avoid getting the thread closed due to being off topic..
  5. I was not able to ask her about it because she told me that she did not want those who did not believe in her.. But she was aware that it was written in her chart that she was single.. (she was a retired nurse) ..to begin with, her main delusion was that she had a chip implanted in her brain so that these JFK followers (of bad CIA agents) can get information about her.. that was what the staff of the institution new, as well as my clinical instructor (since she did not personally interview clients, it was up to the students, she just knows the diagnoses).. Further nurse-patient interactions brought me to a continuation of the delusion that there was a neurosurgeon in the hospital she last worked at, whom she suspected to be the one who implanted the chip.. Then she told me that this person was the one being protected by the JFK followers and that this neurosurgeon stole money from her bank account.. ..i tried to explore more by asking, "How much was taken?", "Were there money left?" She told me that only $5,000 dollars were taken.. I asked, "Can you recall an instance that you may have bought something?", and "Can you recall if you have lent your family members that money?" She continued to tell me that it was stolen.. Then, she refused to tell more because she said that it was a long story and that i should focus on the nursing care.. ..succeeding nurse-patient interactions were more about her daily activities and childhood development..

  7. I'll see to it that the concern reaches my former clinical instructor since it has been 2 weeks when we finished our psychiatric nursing rotation.. ..however, she still is the instructor there for a new group.. rest assured, I'll inform her..:) thank you!

  9. Thank you ma'am psychRNinNY.. yes, I've followed their suggestions during the course of our psychiatric nursing rotation..:) ..i hope your suggestions would be of great help to those new to the experience.. I too had a hard time at first but learned to cope..:)
  10. I'm speechless.. ..i can't comment anymore now to the institution, since our rotation there is done.. and we were only affiliating..

The situation was like this.. ..it was the facility's policy to give proper punishment for those who offend, reprimand, which was done by "facing the wall" - this is given when a client acts violently (verbally or physically) or in a harassing manner (physically or sexually) against another client with no injury meant.. if there was harm done, clients were restrained by tying their hands and was not allowed to join ward activities although can still be interviewed unless he/she was disturbed.. ..for some of those who recovered, they worked in the facility as staff, usually as lecturers.. with those buffed guys, they worked as safety keepers.. these people are those of the higher societal classes and chose to volunteer their time in turn for their recovery.. ..i hope i was able to make some things clear..
  12. well, we just followed the facility's rules.. i think it was the psychiatrists' or physicians' decision to use the tag trainee.. it's a private facility and let's leave their practice up to them..:) it may have been utilized because those with promising prognosis were disciplined, having reprimands or equivalent punishments to learn from their mistake(s)..
  13. They just did not want to address the clients as patients..:) ..it's a privilege of the facility that those who recover, can and may work at the institution as staff..:)
  14. I apologize if I was inactive for too long.. The case study for the rotation really ate my time.. This would be the first time I would be able to sleep properly after a week.. ..Now here's my feedback..:) I did not challenge her.. However, I tried to know more of her delusion, hoping that her schizophrenia was triggered by extrinsic factor/s.. The facility did not provide the old charts, which tempted me to dwell with her delusions, though i did not agree, instead, I tried to explore.. But when delusions are getting deeper, I diverted the discussion to topics with their daily activities and her personal experiences.. ..our rotation for psychiatric ward was over last January, 14, 2009.. I'll miss the experience.. We prepared an activity for them, it was called grand socialization.. The trainees (term for clients in the facility) enjoyed..:) It was a fulfilling experience.. Before returning to their respective rooms, my client thanked me and even bid me goodluck..:) ..I would like to thank all those who supported me through this thread.. Unfortunately, we'll be moving to medical-surgical pediatric clients this coming rotation.. see you again next next next semester when we will return to psychiatric nursing rotation..:)
