VA - Patient Centered Medical Home
Thanks for the reply jeckrn... I, too, used to do home care, but in Manhattan. This "patient centered home" is a new pilot program for the VA (check it out on the web). It attempts to restructure the health care delivery system. Its a very innovative concept and appears to be the delivery system of the future. My role will be as CCM for a localized vet population. Maybe its so new that there may not be others out there who have been introduced to it yet. Thanks again!

Hi! I was wondering if anyone has had any experience as a RN CCM in the VA system that is currently using the patient centered medical home model of practice. Can you describe some of your duties? How are the patients responding to this model? How do you like this model? What do you identify as Pros/Cons? Thanks for your feedback.

That's good to know. At least I can save some money on the course. But I guess that brings me round again to trying to find an answer as to whether or not my eight years in home care counts as a qualifier. Are there any books or study materials you can recommend? Thanks again for all your help!

Hello. I would like to apply for CM certification. I have looked through this thread and still have questions I was hoping some seasoned CMs could answer. My background: BSN, RN for 16 yrs, Med-Surg, SICU, ER, Eight years of Home Care, Six months as Telephonic concurrent review CM for HMO and six months as Short Term Disability Clinician for insurance co. I am currently unemployed. I have researched all the credentialing sites and still have questions. I contacted one of them but have not heard back. Questions I'm hoping you can answer: 1) Does my background qualify me to register? I cannot afford to forfeit a registration fee only to find out I do not qualify. I have taken some sample questions offered by these sites and have done well. 2)Must I be currently employed? 3)Which credentialing agency would you recommend? There seem to be dozens! (CCMC, Kaplan, American Institute for Health Care Professionals, etc.) 4) What books can you recommend? 5) Must I actually take a course or can I just register to sit for the exam? Are the exams given independently or are they through the company that offers the courses? Any and all help is greatly appreciated! I am at a transition period in my career and would like to pursue CM as I feel it is the future for health care. Thanks to all!

Thanks. I do want to get my CCM and have looked into the requirements. I need clarification on whether I need to work as a CM before I can take the courses, or, if my extensive Home Care background meets the requirement. Any input would be appreciated. I did take some sample questions (for what its worth), and aced them. I've reached out to CCMC for more info, but any additional tips from you fine folks is always welcome. Thanks.

Can any one help with salary range?
Thanks for responding. From my research, I am getting a range from $68K to $78K in NJ. I was just wondering what the averages are. When I worked for HC it was a per diem position, paid per visit, and I worked four days a week averaging around $60K. No salaries have been discussed yet but I don't want to low ball myself or seem overly agressive. Thanks again!

Hi, I am being offered a position as a Case manager for an insurance company. The position is in Newark, NJ and my duties include telephonic medical management of members. I have no idea what the salary ranges would be in this local for a BSN with 16 yrs experience mostly in Home care. Any input would be appreciated. Thanks!

Yes, I was looking into CPHQ (Certified Professional Healthcare Quality certification). Disease management seems to be the thrust of healthcare reform and I think it would afford me the opportunity to still impact my patient's lives. The problem is, when I look at case manager postings, they mean different things to different companies. Any key words that companies use to help me narrow my search along the career lines I'm referring to?

Thank you for sharing your experiences. I have been searching for a case management position that will allow me to teach and advocate for patients without direct patient care (and all that traveling) as I did in home care. I feel that's my forte. Any ideas? Thanks for all your advice and insights.

Thanks, I have. I was just looking for some feedback as to the differences in CM positions. Relatively new to CM except for my homecare experience. I would imagine working for a hospital or an insurance company would entail different skill sets and I was just wondering if anyone has worked either and if they could elaborate on their experiences. Hi, I was wondering if I could get some clarification on the CM duties for and insurance Co. like AmeriChoice and the CM duties at a Hosp. I have 10 yrs as coord of care for a home care agency, ICU, Med surg, STD and Concurrent review. I've been a nurse for 16 yrs. It seems the title CM can mean different things to different organizations! My position as a concurrent review RN was also called case management. Can anyone help me understand the different CM positions? Any enlightenment would be appreciated. Thanks

Anybody work for Horizon BCBSNJ? - Opinions?
Hi! I was wondering if anyone has worked for Horizon BCBSNJ and what your experiences/opinions might be? I have the majority of my 16 yr RN, BSN career focused in home care, also worked ICU and had a brief stint as a concurrent review RN in NYC and STD management for insurance co. They're posting positions for concurrent review and RN 1 - does anyone know the difference between these positions? They appear to be very similar in their copy. Are there expected daily case load quotas? In short, what's it like working for this company? Thanks for any insight!
