Assessment - seriously what are you supposed to do?
You have to jump in there and ask other nurses for help or to show you things. If you feel that your instructor is not teaching you things you should confront her and ask her to show you. It was required that every patient we had in medsurg had a head to toe assessment done and documented the day before clinicals each week.

Respirations
After taking the patient's pulse, you can act like you are still taking the pulse but actually count the respirations. This way the patient doesn't know your taking his/her respirations (like the other response said, they will change their rate if they know they are being watched.)

IVPB Question
I am having a hard time grasping the whole IVPB thing. I understand what to do when NOT using a pump (hang the IVPB higher) but my question is: 1) When using a pump, do I put the primary tubing in the pump or the IVPB tubing into the pump. I am assuming the IVPB. 2) If you put the IVPB tubing into the pump, do you connect the IVPB tubing to the primary tubing at the site closest to the patient if there are several ports? Thank you!

Nursing Dx
Hi!! I am trying to come up with a nsg dx for a patient with hypertension. Could I do "risk for injury r/t increased peripheral vascular resistance secondary to htn. Or "Decreased cardiac output rt increased peripheral vascular resistance secondary to htn aeb blood pressure of ...... " Im just not sure of how to make htn into a nsg dx. Your help would be appreciated!!!! Thanks!

I have no idea. That is why im confused. In the chart from today it showed where they flushed after the infusion was over with 20 mL. Im so confused!!!!!

Hi everyone! I am currently doing my pediatric clinical and I have a dosage calculation that is driving me crazy. I have clinical tomorrow and if I dont have this correct, my instructor will send me home with a zero! My patient is a 12 yr old with an INT IV in his left hand. Here is what the order reads: IV sodium chloride 0.9% 50 mL 100 mL/hr Clindamycin 580 mg IV q 6 hours I have already figured out my safe dose range and it is within safe range. The recommended final concentraion is 18 mg/mL The dilution calculation I came up with is 32 mL According to my handbook, the drug should be infused over 10-60 minutes HERES My questions I cant figure out.... 1. What rate will be set on the pump and 2. What volume will be set on the pump? The normal saline part of the order has totally thrown me off!!!!! Please help!! Thank you so much!!!!
