  2. It is not a homework question but some over thinkers preparing for a skills competency. The instructor told us to have the client stick out his tongue for a neuro assessment. I think if there is a right sided stroke symptoms show up on the left. The tongue will deviate to the left due to unopposed action of left muscles. Another person said that since muscles only contract, the tongue would go to the right because there is only right sided strength. I can't find this kind of detail anywhere
  3. if a patient has a right sided stroke and exhibits left-sided weakness, if he was to stick out his tongue for an evaluation of cranial nerve XII, the hypoglossal nerve, and considering that muscles only contract, what side of the mouth would the tongue stick out? Thanks for any input

  5. pH 7.25 (Acidosis) CO2 50 (Acidic) HCO3 27 (Alkaline - but going in opp direction of problem - partially compensating) Partially compensated Respiratory acidosis How do you know this is respiratory or metabolic? I thought the 'off' item would be in the same direction as the pH. Thanks all

  8. This seems to be such a vague diagnosis and difficult to find rationales for. Besides searching all over the Internet for potential evidence-based practice, where do I find rationales for my nursing care plans. The books I have give rationales for cases that don't apply to mine. Can someone suggest the best way for me to find rationales? This is only my first semester, and I'm hoping to get organized, better, to help the next semester go more smoothly. Any books or databases or ideas would be greatly appreciated.

  14. How do you figure out what is the related to and what is the as evidenced by part?
  15. Can someone help me figure out how to write the care plan. If I have tissue perfusion ineffective cerebral, what is the concept or rules for the R/T and AEB? Do I change the order of the words to be Ineffective cerebral tissue perfusion related to what is the cause and AEB what are the symptoms? Thanks for the help
