  1. I PASSED!!! I am so happy. I screamed and believe me everyone in the office heard me.
  2. I just took my nclex LPN and it stopped at 132. I am so scared that I failed. Has anyone stopped at 132 and passed.

I spoke with her a little bit about her constipation she said that she had surgery when she was five to remove a fist size blockage in her and she has always had problems every since she was little. She is not anorexic she weighs 90 lbs and her muscle tone is good. She did have a bit of a distended stomach and it was hard. She told me also that she doesnt go to the bathroom except maybe once or twice a week and it is usually very hard and hurts and that is why she is on the medicine, which of course she says she hates. Thank you very much for the help.
  5. I have an 11 year old patient with chronic constipation. I know her through my peds rotation at a school and she is constantly coming in for stomach pains because of constipation. It is marked in her school records about the problem. I have do do a care plan on the young lady and am a little confused at where to start when I do not have access to a lot of info on her. All I am able to get is sharp pains across the back flank area and in the lower abdominal areas. This start off sharp and then ease off a little. She is on miralax and does have some leakage and has to wear a pad for the leakage some times which does embarrass her at times. Thanks for your help
  6. I am having a little bit of trouble putting together a mental care plan for my patient. Any help would be appreciated. He is diagnosed with schizophrenia. History of suicidal attemps, attacks on people because of audiotory hallucinations. He hears voices all the time. He does not understand his illness. Lost job because of attacks. He is an elderly man and does not have any family support. THank you very much for any help.
  7. Can afib cause an elevation in creatine on an electrolyte test. The creatine is 2.0 (nv - 0.7 - 1.4mg/dl) or can heparin or coumadin therapy. THank you for any help

  9. I am needing help with nursing diagnosis for my care plan. My patient is 51, in good health. The only problems he has had is history of hypertension. He was admitted to the hospital after complaints of fatigue, pain in the lower legs, and fluttery feelings in his heart. He was diagnosed with Afib and a clot was found in his heart chamber. all of his CBC and electrolyte test are normal. His Ptt is 100 and INR 1.7. He is on Coumadin, Lovenox and Coreg. BP has been around 113/74 because of the medication. It was 145/95 when he was admitted to the hospital but has been reduced. Coumadin and Lovenox are new medications that he has just recently been put on because of the clot. Any help is appreciated

  12. If anyone can help I would appreciate it. In our careplans we need to try and understand what can cause lows and highs in different areas. I have a patient who has a right middle cerebral artery, she as a bladder infection, she is on a foley catheter, she has hypertension, etc. The one thing i could not figure out was elevated Eosinaphils and abs. Eosinaphils. I know that these are usually elevated in the event of a parasite or allergic reaction. There has not be any reference of either in her chart. Does anyone know what else can cause the elevation. Usually the neutrophils and abs neutro. are usually high with a bacterial infection. I am just a little lost on this. Thank you for your help.

  15. Thank you very much for the ideas. I like the idea of presenting it doing it in the wrong manner and then explaining the correct way and talking about what was wrong with the first presentation.
  16. I am peer teaching about surgical asepsis and I need some creative ideas on presenting this to the class. Any sugggestions would be great!!
