Care plan
I have to develop a careplan for a patient and am not sure what to relate my diagnosis to or what to do for evaluation. I have put down a related to statement but I am not happy with it, what do you think? Thanks Diagnosis 1) Risk for unstable blood glucose related to lack of motivation to monitor diabetic status Goals - Patient will understand the importance of monitoring blood glucose level - Patient will monitor blood glucose before meals and before bed. - Patient will exercise on a daily basis (walk at least 20 minutes a day) Limit sugar intake and eat a well balanced diet Interventions Teach patient complications associated with diabetes (hypo/hypertension) - Ensure patient understands the importance of monitoring blood glucose level Teach the patient the importance of exercise

HELP PLEASE!!! Wellness Dianosis
I have come up with 3 nursing wellness diagnosis for my client and family, and am confused as what to related them to, my ideas are in brackets, but I am not sure if it is the right idea. My diagnoses are 1) Progressive Transition to extrauterine life. 2) beginning intergration of infant into family (related to relationship with older siblings) 3)increasing confidence in infant care skills. (related to practice of skills) Help..please!!!

Goal Time Frame???
I am trying to time frame (goal is to reduce his blood pressure to below 140/99mmHg). He is going to be implementing exercise, DASH diet, and appropriate rest and relaxation. That being said, would it be appropriate to set short term weekly goals of decreasing blood pressure, and a long term goal to get below 140/99mmHg with a time frame of I do not know maybe a couple of weeks or a month... ( i do have references that tell me that the change in his diet should reduce his blood pressure by b/w 8-14mmHg, and exercise reduce his blood pressure by b/w 4-9mmHg, but I have no time frame)

Goal Time Frame???
I am writing up a care plan for a patient with htn and am having trouble coming up with a good time frame for his long term goal to be achieved by. His goal is to reduce his blood pressure to below 140/99mmHg. He is in a case study so I do not know what his actual blood pressure is right now, I just know that he is hypertensive. I have tried looking it up but I cannot get a straight answer, and I do not have enough experience as of yet to make an estimate. Any help would be appriciated... Thanks

Thanx...I did look up the symptoms of htn, but I did not see how it would apply to nursing diagnosis, because they were all related to the medical diagnosis...The nursing diagnosis handbook I have doesnt related anxiety and fatigue... I really appriciate your help, and would really appriciate if you could refer a good nursing diagnosis book...I think I am good to go now...Thanks again!

Thanks...I think knowledge deficient will work...but I still need one more, my problem is that all of the information above is all I have to go on... it was given to me in a case study... the only other information i have is that he doesnt take much time to relax, but he usually gets a full night rest (6ish hours) because he is ethier working or spending time with his kids, and he doesnt exercise very often...

This male was diagnosed with hypertension 2 years ago, but has notbeen put on any medication. 2 months ago he went to his family doctor complaining od of fatigue, headaches and dizziness and was reffered to a blood pressure clinic. Upon assessment he is very tense, restless, as well as very talkative. Due to his situation at home I have come up with the diagnosis of anxiety, however I need at least 2 more and am having trouble coming up with these...could someone please help!!!!?
