What should my nursing dx be for my patient whose platelet count is 63 (related to bleeding risk)?!?? Any suggestions?! Thank you!!

I agree with your preceptor, but it is up to you and something to discuss with your OB doctor. I am 14 weeks myself and plan to have a doula for support. Your chances for a c/s drop significantly by just having a doula. I hope to have a natural delivery myself.. this will be my first one and hope it doesn't come to the point of having a cesarean. Hope everything works out for you!

pt post cardiac cath
Hello~ I recently was taking care of a pt in the cardiac progressive care unit who came in with syncope and atrial-fib. A heart cath was later performed that day and results showed blockage of all 3 arch vessels so a CABG was scheduled later in the week. I have come up with a list of nursing diagnoses for this patient but am having trouble writing out my care plan for the top three. So far I have my first two written (decreased cardiac output and altered cardiopulmonary tissue perfusion). Pain was also a focus this day as she was having much pain in her lower back (but not relating to her dx). I think risk for infection should be one of the top priorities b/c of her wound assessment post heart cath (she also has an elevated WBC count). She also is at risk for bleeding.. can't really be used as a nursing diagnosis though..?!? Anxiety and activity intolerace were some other dx I addressed this day. I just can't seem to pick my last top diagnosis! Any thoughts/advice??! THANKS :wink2:, confused & concerned nursing student

Thank you on your input! I actually used ineffective tissue perfusion as her priority dx. She did have a decrease of 6% of body weight today, but I realize that decrease is expected in the first week of life. However, she did have some jaundice today (in face & upper chest), but her total bili came back normal (10.6). She got dismissed today & didn't exhibit acrocyanosis or bruising today either. Do you think that tissue perfusion should have still been my priority today?? Maybe more breastfeeding to rid the body of the jaundice??

I am questioning now about my baby's priority diagnosis!! She has more data including: acrocyanosis (was born yesterday and still has!), irregular heart rate and rhythm...the dr. noted this as typical sinus arrhythmia and is considering doing chest x-ray and EKG), and has brusing on her labia majora. She is getting breastfed and seems to have no problems with that. Her head AND chest circumference are both 34 cm (head should be bigger..I'm wondering what problems relate to this); weight 3166 g and had good Apgar's (7 at 1 min, 8 at 5 min). Do you have any nursing dx suggestions??? I am more confused about this one!!! Thanks!!!
