I don't care how many patients/ residents there are I would want to know the BP if I was giving the med. Id have the CNA's get the vitals if there low hold the med I find as a new nurse most new nurses try to do things the way we are taught. The problem is when everyone is used to doing things the wrong way -they will have a problem with you for doing it the right way. If you cause any harm doing it the wrong way they won't be there to back you up in court.

Okay so what do I do, im not going to write orders for the doctor but my DON keeps telling me too

This is in an ALf with an ECC/LNS license-And the Don keeps making statements to me that Im not writing up telephone orders for things that the doc has said we could but I dont feel comfortable doing it.I want the doctor to actually look at the wounds and make a decision.I feel like Im being asked to do his job.I asked if we could get standing orders the DON says its not allowed at the facility. Is there some nurse practice act to back me up that I could make a written statement and refer to

Have any of you been asked by your DON and the doctor to write telephone orders for things like home health for wounds,Xrays for falls/injuries and UA's if you suspect a resident has a UTI without actually calling the doctor when these things occur and the doctor will sign the T.O when he makes rounds No standing orders allowed at facility

This question is for any Nurses really up on Nursing documentation. I'm a new nurse, I got a pocket guide book from hcmarketplace that described how to chart care provided by another nurse if that nurse calls and asks you to chart an entry she forgot to do. The book stated after you write the note to sign and then have that nurse co-sign. Well I did this yesterday and when I went back to work I had a message from our DON to draw a straight line through it and write mistaken entry/he stated it was not legal to chart care provided by another nurse. Now the book says you can....So I was just wondering has anyone else done this and know anything about this kind of documentation or is it true that it is not legal?
