NATIONAL COUNCIL OF STATE BOARDS OF NURSING, INC. NCLEX® FAILURE CANDIDATE REVIEW AND CHALLENGE AGREEMENT I agree to observe the following conditions during the NCLEX review and challenge: 1. I understand that I will review and have the opportunity to challenge the test items that I answered incorrectly at the Pearson Professional Center, located at 3390 Pine Tree Rd., Lansing, MI. 2. I understand that in order to be admitted to the test center I must bring the Authorization To Review, photo identification with my signature and another identification with my signature on the day of the review and challenge. My name on the Authorization To Review must match the name on the photo identification. I understand that I will not be admitted to the review without these documents and will forfeit my review and challenge appointment, and will be required to submit a new request and pay an additional review and challenge fee. 3. I understand that I will be admitted to the review and challenge in a manner similar to how I was admitted to the examination. 4. I understand that I will review the test items in a room with a representative from the Bureau of Health Professions present. 5. I understand that I will not be permitted to photograph, copy or record any part of the test items. All notes I take must be on paper provided by the test center staff. These notes will be destroyed after the review. 6. I understand that my time for review and challenge is limited to 2.5 hours. If I was granted extra time during the examination as a modification for a disability, then I may have one-half the time originally granted. 7. I understand that I will be allowed one review and challenge per examination and that I must review the most recent examination taken, and that I MUST complete my review and challenge prior to registering to retake the NCLEX. 8. I understand that I will be allowed to bring nursing textbooks into the review to be used as references. 9. I understand the decision of the National Council's Board of Directors regarding a challenge is final. 10. I will submit the fee required to review the NCLEX. A separate fee and form will be required if I decide to challenge the NCLEX. I understand that this fee is not refundable under any circumstances unless my challenge is upheld by the National Council Board of Directors. NCLEX EXAMINATION REVIEW PROCEDURE FOR MICHIGAN The state of Michigan permits review and/or challenge of the NCLEX® examination. A representative from the Bureau of Health Professions of the Department of Licensing and Regulatory Affairs will be present during a candidate’s review session. This individual cannot answer any questions about the items or provide additional information about examination content during the review and challenge process. BEFORE THE REVIEW 1. Reviews must take place within six months of the release of the candidate’s examination results. In order to review an examination, a candidate must submit a properly completed REQUEST FOR NCLEX® FAILURE CANDIDATE REVIEW AND CHALLENGE form, along with a properly completed NCLEX® FAILURE CANDIDATE REVIEW AND CHALLENGE AGREEMENT (enclosed), and the review fee of $400.00. All fees must be paid with a money order or certified check (personal checks will not be accepted) made payable to the National Council of State Boards of Nursing (NCSBN). If a candidate has already registered for a re-examination and sends in a $400 review fee, the candidate will not be allowed to review his/her prior examination and will forfeit the $400 review fee. Submit the completed forms and fee to: Lucinda Clark Exam Reviews P.O. Box 30670 Lansing, MI 48909 2. Upon receipt, the properly completed forms and the review fee will be forwarded to NCSBN. NCSBN will in turn fax a copy of the Request to Review to Pearson Professional Testing (PPT). PPT will then send the candidate their Authorization to Review. The Authorization to Review will include information about how to schedule the review appointment. 3. Reviews will be conducted at the Pearson Professional Center, 3390 Pine Tree Road, Lansing, MI. THE DAY OF THE REVIEW 4. To be admitted to the test center, the candidate must produce an Authorization To Review (which will be sent to the candidate from Pearson Professional Testing), a photo ID with a signature and another ID with a signature. 5. The standard review and challenge time period is 2.5 hours. If a candidate was granted additional testing time in which to complete the exam, the candidate's review time will be half the total amount of time that was granted for the examination. If a candidate tested under special conditions that required assistive personnel, the same conditions will be available for the review and challenge. 6. The candidate may review and challenge only those items answered incorrectly. The correct responses to these items will be provided, as well as the candidate’s responses to the items. 7. One review and challenge per examination is allowed. The candidate must review and challenge the most recent examination taken. The fee for a challenge is $90 plus $110 for each item challenged. 8. The candidate may take textbooks into the room to serve as references during the review and challenge. All notes taken during the review and challenge process will be recorded on paper provided by the test center and will be shredded upon completion of the session by the test center administrator. 9. No photographing, copying, or electronic record shall be made of the examination content.

Hello, I failed my NCLEX-RN exam 3 times. Between the second and the third time I had exactly the same result, which is "near passing score". Now I have no idea what to do: how to study and what to study. I took HURST (before my second time) and Kaplan (before my third time). Now, I would like to see my actual test, to analyze my tests and study properly for the next time. Does anyone know, if it is possible to review your actual test? I called Bureau of Health Professions, and they said that you need to pay $400.00 to do so. Does anyone know if this is legal to charge this money to review you own exam?! Thank you! Paul

thank you, for your quick answer. i do have nursing degree from russia (ukraine) i do have citizenship in america i do have bs degree in america (business and finance) do i still need to pass toefl exam? i understand, that i need to do evaluation of my diploma, then i need to do bsn (12 month) and than i need to finish two years crna school (24 month) plus one year of experience in an icu unit. am i right? thank you!!!
