I posted earlier a comment describing my situation in the ER ( Subject: ER Staffing) I realise that in Quebec, level 1,2 and 3 are the exact opposite of what I read from the US. A level 3 hospital (if the translation is correct from Centre tertiaire, in french) is the one with 24h readily available specialists of all kind... whereas the Level one -centre primare- is the one who can stabilise the critical pt but has to transfert it to level 2 or 3 for more specialised care. I work in the province of Quebec, and few years ago, we (ER nurses) obtain from our government to be paid the same premium per shift as the Critical care nurses. Our statement was: our patients are unstable, face lifethreathening situations, and we are the overflow of the ICU anyway so we also have to deal with intubated patients ( and not on a 1:1 ratio, as you know) so yes, as an ER nurse in Quebec I'm considered as a critical care nurse and dare anyone to tell me I'm not!!! ;P

I've worked in a 22 beds facility--3rd level trauma center--, (fairly busy with >100,000 pt a year, >1000 ambulances a month, ) with 2 triage room, 1 stab room (2 beds) observation room, trauma and pediatrics... we used to staff on a 3:1 basis, had 2 triage nurse during day and evening shift that would only do outpatients triage (at night, the nurse who as affected to minor trauma and ambulance triage also does all the outpatient triage) 3 floating nurses.... we were pretty well staffed. Now I work in a small 7 bed facility and we are poorly staffed. Would you believe at night, there is only 1 nurse. No receptionist or anobody else. You carry your blood samples to labo yourself, you open your patients files, you do triage and floor in the same time and you clean your stretchers when the patient is discharged. Sounds safe for the patients? not likely....

I had to give Lovenox IV lately in stab room in a case of acute coronary syndrome. I found litterature for that specific case.
