What is the worst thing you see on your floor? I always hate when people think OB nurses don't see bad stuff. For me the things that freak me out the most are HELLP, DIC x1, and shoulder distocias. We had one patient with a liver rupture which i guess if very rare! If I remember right she didn't have any prenatal care. She actually lived.

Didn't think DKA cause glucose was wnl. Didn't want to think hellp cause liver enzymes were ok even though platlets were lot. She got a diuretic today and started having good output. Still a 1:1 care. got more blood. I didn't care for her today but had to check up and make sure she was still ok.

renal is my concern but I don't know if it would be from a hemorrhage. Really concerned for her healing post a surgery. also pre eclampsia

what would cause such a low albumin level? I dont want to give much info about the patient but they had severe edema, creatinine of 1.1. very little urine output. alt 300 ast 25. bun 11 I left work today and am still worried about the labs. Dr. not so much.

do you get paid more for certifications?
I agree that certain certificates like NRP, BLS are to be expected. I wouldn't expect anything for that. But how does it benefit me to get the BSN or RNC-ob or NCC efm?

How long did it take you to feel like you knew what you were doing?
I'm having days during orientation where I feel like I've got it. Other days I feel like its my first day ever as a nurse and I feel lost.

$22 and that is with 2 years exp and on night shift!

Day shifts hard to find for new RN grad w/i no seniority?
In this economy I'd be happy getting any position. I wouldn't worry if it was nights or days. i would just try to get the experience

How long did it take you to feel like you knew what you were doing?
I've only been doing L&D a few months. I'm so anxious I'm going to mess something up. Not call the dr soon enough/call them to early. How long did you do L&D before you felt like you knew what you were doing? I heard it takes 5 years for them to consider you experienced. I'm hoping I feel a lot better after the first year!

do you get paid more for certifications?
They want us to have all these certifications but dont want to pay us for having them. We will soon be required to have NCC for fetal monitoring. They are strongly encouraging RNC. Just wondering if you get paid more at your facility for having certifications.

Orientation for newly-minted grad
I'm not sure what a SNF is...But only one and a half weeks of orientation is not very nice! That is not good especially for a new grad. I got 12 weeks orientation as a new grad.

New L&D Nurse
I've been a L&D nurse for a whole two months now..lol I think the thing that helped me the most is reading up on Fetal monitoring before I started my orientation. I read the entire AWHONN intermediate fetal monitoring book before I started. I think that really helped. I have 4 weeks left of orientation. Its a crazy time but so worth it. I really love doing labor! If you've worked on another floor for a while that will help a lot. We had a completely different charting system that I had to learn.
