  1. We use MediRecon (http://www.medirecon.net) and the ER does the starting medication reconciliation online. Therefore, no forms to deal with, and its easy to use. In general, it is the duty of the ED to do their part, but many ED's will just setup the home med list only. Not much more than that.
  2. All of the EMR systems we have looked at do not know what to do; the shared care plan is cool, but the medirecon.net does a great job with meds and med recon, has its own database.   3. At my institution we are doing community based med recon and they have a great program that the patient can even logon to. The doctors logon to www.medirecon.net and the patient logs on to www.mymedilist.com. The forms are good, but this system even lets me print personalized wallet cards when we discharge them. Give it a try. I think they are not too expensive.
  4. We use and electroinc form at our 4 person site. www.medirecon.net. We need to use this due to the fact that we have older patients and this electronic system allows us to print wallet cards and everything.
  5. We are trying a system that has both outpatient and inpatient medication reconciliation. They have a great proactive site. We have a 4 person site. :monkeydance: I fee like a monkey juggling things some of the time. But this system is cool. I think other people should try it. www.medirecon.net. We have alot of older patients and they need a list and this site does this. MK
  6. My hospital is looking into an electronic answer to this, and we found only a couple of guys doing this. One is www.medirecon.net. The have an electronic version of the reconciliation. makes it real easy. We are going for it with this group. MK
