  1. chare's post in How do I write a personal letter to the Board? was marked as the answer
In 1981 I was charged with DUI. When I appeared I pled guilty and was granted a stay of sentencing and allowed to participate in an alcohol and drug safety action program.  Upon completion, I again appeared before the court and was found guilty of the reduced charge of reckless driving.
In 1998, when I applied for licensure with the WV BRN, the question was worded such that I was required to report the conviction.  Like you, when I contacted the court I was told that the record was only maintained for 10 years and the clerk of the court provided me a statement explaining such.  Which I submitted along with my personal statement.
I wouldn't concern myself with the length of the document.  I would ensure that is was an accurate and detailed accounting of the events, to include disposition by the court, what you learned, and, what you've done since then; emphasizing that you have had no further judicial intervention since.  And, most importantly, keep a copy of everything you submit as you will likely have to do this again if you apply for licensure in another state.
As this is a 23 year old misdemeanor, I suspect you'll have minimal difficulty.
    Best wishes.
  2. chare's post in Dosage Calc IV Infusion Question was marked as the answer
If you have to administer 40 mEq of KCl over 2 hours, how many mEq hour are you going to administer?  Once you know this, you can easily calculate the hourly infusion rate.  I don't know how you are calculating this, but would encourage you to learn dimensional analysis.

  3. chare's post in IV Fluid Replacement Question Debating Book Answer was marked as the answer
To determine daily pediatric maintenance IV fluid requirements you can use the following formula:
    1 - 10 kg: 100 ml/kg/day
    11 - 20 kg: 1000 mL + 50 mL per each kg >10kg
    21 - 30kg: 1500 mL + 20 mL per each kg >20 kg.
    >30 kg: 1700 mL + 10 mL per each kg >30.
Using this formula, the daily IV fluid requirement for a 19.5 kg patient is 1475 mL (1000 mL for the 1st 10 kg, then 50 mL per kg for 9.5 kg). Dividing by 24 hours, the hourly infusion rate would be 61.5 mL/hour (62 mL/hour).
Alternatively, to calculate the hourly pediatric maintenance IV fluid requirement:
    1 - 10 kg: 4 mL/kg/hour.
    11 - 20 kg: 40 mL/hour + 2 mL/hour per each kg >10.
    >20 kg: 60 mL/hour + 1 mL/hour per each kg >20.
