What is your practice in identifying a pt being high risk for falls?’ For example, questions you are using, screening tools, information you want to know from senior pts,etc..
  3. Hello, I am doing a project, and I would like to know what is your opinion in why so many falls occur in the geriatric community. 1. Why geriatric patients that present to the ER with a complaint of fall at home end of falling while in the hospital? 2. What information do nurses need to know in order to prevent seniors from falling when triaging? 3. What information do patients need to tell the nurses or health care provider to prevent falls at the clinical setting? 4. What are we missing to prevent falls even if the resources are there and why?. PLease help!!
