    I've had trouble with mine too. I sent mine to the CRNA 10 days ago, but he said he never got it. I sent one of those reminders as well today, so I'm going to give it a day or two, but I'm not sure if its supposed to be right away or not.

    Regarding the employment verification form, I guess by employer they just mean your manager can sign it? I wasn't quite sure what that meant, but if your manager is writing a letter of recommendation for you they would obviously be verifying your employment.
