having a hard time coming up with diagnoses for my nb postpartum care plan.... -Pt had a serum bilirubin level of 11.4 mg/dL. -Was put on phototherapy. -Not breastfeeding well: mom has short/flatter nipples, the baby has a poor sucking reflex/not latching -sunken anterior fontanel -7 lb 2 oz; 19 1/2 inches long; 32.5 cm (head circumference) and delivered at 37 weeks and 5 days would it make sense to do something to do a dehydration diagnosis? or is a sunken fontanel in itself not enough evidence?
