On the physical exam form? I printed it and let my doctor fill it out. Everything is filled out except for meningitis and hep a&b combo. I need to get my second MMR as well. I was told it has to be a month apart.

In the email from HCC it said that identogo would contact me by email to get it done. My email from identogo said i was pre-enrolled for it.

I got an email directly from them saying i was pre-enrolled

I did the titer for Hep C

Once you receive the email from identogo you’ll make an appointment to do your fingerprinting. After you do your fingerprinting you’ll receive it in the mail in a few days.

I got it in the mail

They’ll send a blue card. I received mine today.
