Yes I've done all the immunizations. Well I was accepted to another nursing program before csub and I used the form from that school and used it for csub and it was accepted. However, if you type in csub nursing physical exam form on a search engine, a link does come up that has a physical exam form for csub. Im sure you can use that since mine from a different school was accepted. Or Im sure your doctor has a generic form that will work as well. As far as immunizations, if you have them whatever proof you have should work. If not, request your doctor for titers or the shots themselves.

That's very common with nursing programs, I would recommend you accept it as a backup just in case.

Has anyone been accepted? I'm told email acceptance letters will be sent out starting this week and next week and all the way to August of course.
