I asked for a format for a summative review report and they told me there is no required format. Keep in mind the purpose, which is to provide evidence that you completed the plan action. It should be no more than one page and clearly identified with your name and the action of your plan that it satisfies. I would also suggest that you also follow up with cgfns to ask whether when will your credentials expire just to make sure you won't be wasting time.

Hi Blinker, I applied for New Mexico and a retaker. I submitted by remediation and recently got approved by the BON to move forward for my reapplication. I was wondering if you have any idea if I need to submit requirements or do the whole process again? I would really appreciate your response. Thank you
