  1. I would say that yes this is falsification of documentation and more than likely failure to follow policy/procedure. As for consequences will depend on the organization. I am one that believes in mentoring and coaching. Has this person received any of this? What is their work ethic like? It does need to be moved up to leadership though. It is the right thing to do.
  2. MSUOUgirl I have never done psych but I can say that it is never okay to yell at a patient - even if you are being yelled at. It seems that there might be some burn out happening with your preceptor. I would definitely say NOT to let this go. Not knowing the entire situation but did you ask her why she responded to the patients that way - she might not even realize she is acting this way. As a leader in nursing - I personally would want to know that this was happening in my department or my organization. The only way things can be fixed is if leadership is made aware of it. Maybe suggest to your manager education regarding communication to patients. You can still share the experience with your manager and not give any names. I hope this helps.   3. I am a new Chief Nursing Officer for an FQHC. One of my first responsibilities to create an education/orientation for all new LPN/RMAs. I have a few questions: How long should orientation be for a brand new RMA/LPN in a clinic setting? Does anyone use IMS EMR? Does anyone use online education software (i.e. Healthstream, Elseiver Clinical Skills - others?????)? Thank you Mike
