I am a new grad working on a telemetry floor and I have trouble distinguishing vtach on the tele leads sometimes. Don't get me wrong, when I see the typical wide QRS I get worried but sometimes the top lead (II) shows a wide QRS but then the bottom (V) shows a narrow one. Generally I look for a similar morphology in all the leads. Is this correct or is there something I'm missing? Thanks!

When does BP become an issue with sepsis? I understand that septic shock is when the patient's BP doesn't respond to fluids and vasopressors are needed but if a patient has sepsis or severe sepsis when does the BP start to drop?

I just finished my orientation with my preceptor on the progressive care unit. He was a good preceptor, very smart and willing to teach. My question is does a nurse's working memory come with time? He would remember labs, vitals, orders, and other info about patients after hearing it once and I always have to write stuff down. Any tips for getting the memory better? Maybe I'm still lacking in putting the big picture together.
