Hello,
I am currently preparing for NCLEX using Kaplan. my question is what they mean by remediate each question. I just read the rational and look up the topic of the question if I didn't know it . is that enough or is there something else I should be doing?

Yup! Remediate means to read each rationale for each question whether you got it right or wrong. I took an extra step and looked up content that I wasn't familiar with. I'd usually look it up in Saunders because it was short and sweet--Kaplan's content was a snoozefest lol. Good luck!
