Please help with care plan!

I need a nursing DX for teaching a pt cast care instructions (or something along those lines) My pt was an 8 y.o. F, who had broke he right radius and ulna. She was discharged. Please help me I have one more chance to get it right. I am so awful at care plans. :crying2: Does anyone know of any GREAT care plan books at Books A Million? Thanks

Knowledge deficit relating to personal care of cast to ® arm as evidenced by patients age (she is 8 years old) and lack of understanding relating to care of cast.

Goal: Patient will demonstrate proper care of cast prior to discharge

Interventions:

1. Provide instruction to patient as well as significant family member.

2. Teaching about keeping cast clean

3. Teaching regarding s/s of infection (swelling, odor, drainage, temperature).

4. Teaching regarding circulatory checks and what to do it arm swells.

5. Teaching regarding when to call the physician.

The care plan book that I use is:

Carpenito, L.J (2002). Nursing diagnosis: application to clinical practice (9thed). ISBN: 0-7817-3319-7

Actually, this nursing diagnosis is in the book. I had a young patient with a cast and this was the care plan that I used for him. I have been writing care plans in the acute care setting for so long that it comes without referencing the book, but yes, knowledge deficit is in the book I listed.

Carpenito is a great book for students that need information on nursing diagnosis and I think you will find this very helpful.
