Paper documentation help

Are there any good guides out there for help with documentation. I am a home care nurse and still am using paper documents. I have a hard time remembering how to properly document sometimes. I have searched for guides but havent had good luck! Thank you

Because each facility that uses paper documentation is probably going to be a little different in what paper documenting system they use it may make sense to create your own guide. I work float between several facilities that all use paper documentation and I keep a binder of notes on how to document at each facility.
