I have had a patient 98 with CHF and she is extremly anxious with severe claustrophobia. She has PVD with 3+ pitting edema in all extremities...I have many NANDA diagnosis for her. However, I am at a loss for interventions and outcomes. She died on Saturday and my careplan did not get to be finished. My clinical instructor said she was going to grade me on my outcomes more then anything.
So, my question is what are some good outcomes for a pt that is deceased? I was thinking To provide company to client and provide a sitter so she will not die alone. (She voiced concern of dying alone many times) I am at a loss can anyone give me some direction?
