Medication Administration Record, basically the "book" in a loose-leaf binder with pt med info, which I'm sure you guys have too, probably just called something differfent.

I've worked at a facility with the bar-code system. You scan the barcode on the bracelet, and it identifies the pt as the correct pt for the meds to be given at that time. Kinda' fool-proof, if used correctly.

Ah!! A med book!! LOL

Just never heard it called anything else.

Medication Administration Record. (Heard lot:) s of other names for them-what do you call them where you are?)

We do get a printed out bracelet with name, bar code, hosp #, DOB, along with each sheet of pt name labels. We aren't using them for med administration-yet-where I work but we have a glucometer that will scan the barcode and display the pt's data on the screen.

yeah, but we still call it a MAR if it's on the computer and signed off electronically even though it wouldn't be in a book, or even in print because its still the record of medication administration. if you had a hand scanner and individually dosed packet of medication (as we do here). you would scan the pill, scan the patient and it's the equivalent of giving a pill and signing off the time in a big book. just high tech, and it keeps 'em honest. and, that's not everywhere. if you were in a long term care facility, they don't even wear ID braclets.
