Nursing Process: Evaluation

I am confused about the descriptions of goals being "met, partially met, unmet" etc. In the nursing process, what happens if the goal is met? what happens if it is partially met? please explain a little more about the descriptions! thanks :)

In the nursing process you have Assessment, Diagnosis, Plan, Implementation? And Evaluation.

Assuming you understand assessment and diagnosis, you then PLAN your care for your patient and set a goal for that patient.

For instance, a patient who is in rehab may have a goal of "patient will walk 60 feet by the end of shift"

Then you implement the plan, which could be, having the patient walk to the dining room for breakfast an lunch, or walking to the nurses station for a card from a family member (just using this as a suggestion as if the patient needed motivation to meet the goal)

Then you EVALUATE, which would be "goal partially met, patient only able to walk 40 feet by the end of shift"

Does that make sense? Hope this helps, I am on my iPhone and typing is hard for me

Yes; I am pretty familiar with the whole process, but specifically for evaluation I was wondering what you would do if the goal was partially met or not met... for example, if the goal is met you would discontinue interventions for said goal. if it is partially met, do you continue with care as planned or make changes to the care plan? and if it is not met, what would be the protocol then?

but thank you for your help!

Basically you just decide if the goals that you set for your pt have been achieved. For 'partially met' I would say that just means perhaps there was an improvement but not quite all the way there. If 'unmet' you would need to state why not and either adjust your goal so it is attainable or perhaps change the time frame and continue to work on it. Hope this makes sense!
